Please read and check the boxes below to signal your agreement before canceling.

If you reactivate within 6 months of cancellation:

• You must retain your previous Representative ID number, and unless ZYIA ACTIVE determines otherwise.
• You must purchase a new Starter Kit.
• You may not change your sponsor.
• You may not have any of your previous downlines reassigned to you.
• You are not eligible for any new Representative programs or awards from ZYIA ACTIVE.

If you reactivate 6 months or more after cancellation:

• You can’t retain your previous Representative ID number.
• You must purchase a new Starter Kit.
• You may change your sponsor but are not required to do so.
• You may not have any of your previous downlines reassigned to you.
• You are eligible for any new Representative programs or awards from ZYIA ACTIVE.

Customer support will need to confirm the information if the required information does not match. In those cases, a confirmation email will be sent before the account may be canceled.
